Accessing DAMIS

New Submitters:

If this is your first time accessing DAMIS, you will need to create a login.gov account. 

  1. Go to login.gov
    Open your browser and head to the site.
  2. Click “Create an Account”
    You’ll find this option right on the homepage.
  3. Enter Your Email Address
    Choose an email that hasn’t been used with login.gov before.
  4. Confirm Your Email
    login.gov will send you a confirmation email. Click the link in that message to verify your address.
  5. Create a Strong Password
    Make sure it meets the security requirements listed on the page.
  6. Set Up Multi-Factor Authentication (MFA)
    You’ll choose a second method to verify your identity—like a text message, authentication app, or security key.

You’re In! Once your account is set up, you can log in anytime at login.gov. From there, open a new browser tab and go to damis.dot.gov to access the MIS submission portal.

Returning Submitters:

If your DAMIS account from last year has not changed go to https://damis.dot.gov. Click “login” and enter your Login.gov credentials from last year to enter your data. 

If you received an activation code, click "Register" and enter the 32-character activation code to register an email address with DAMIS.

 

Instructions: Employer Information

Once you have accessed your account:

Select Enter or Edit Your Company Data

  1. Enter your company's information:

    • Company Name

    • "Doing Business As (DBA)" Name

    • Address

    • Email

  2. Enter the name and complete telephone number of the company official, certifying the accuracy of the report and the date that the person certified the report. 

    • The company official cannot be a service agent (e.g., representative of a consortium or third-party administrator). 

    • A service agent can prepare the report, but a company official (e.g., safety officer or other authorized company representative) must certify the accuracy of the report by submitting it. 

    • If someone other than the certifying official completed the D&A testing form, enter that person’s name and phone number. 

  3. If you use a Consortium/Third Party Administrator (C/TPA), enter the name and phone number.

    Under PHMSA-Pipeline, select the category (or categories) that match the type of work your oil/gas operator performs.

Covered Employees

Enter the total number of PHMSA employees that fall within the drug and alcohol testing requirements for your company. 

Hint: You can obtain this number from cell N4 from your yearend NCMS stat data report.

 

Drug Testing Data

  • Use your NCMS year-end report to complete the information. Refer to Column N (Totals) to complete each field.

  • Select "Set blanks to zero" for all blank cells.

 

Alcohol Testing Data

  • Use your NCMS year-end report to complete the information. Refer to Column N (Totals) to complete each field. The alcohol numbers will be found towards the bottom of the file.

  • Select "Set blanks to zero" for all blank cells.

Once the data makes it through data validation without errors, a dialog box will ask the user to certify the results, provide an electronic signature, and submit the data. Select the “Sign and Submit” button to complete the report submission. The user will get an option to receive an email confirmation of the completed submission, we recommend you use this option to obtain a completed report for your files.

Important! Verify your information has been submitted by ensuring the green submitted is shown.

 

Special Note: If you return to your DAMIS account and want to view your information, ensure you click the View Your Data (Read Only)” link.

If you click “Enter or Edit your Data”, it will change the report back to Incomplete; If clicked by mistake, you will need to proceed through each page and “submit” the report once again, to receive the green dot completed status.
